THE COMPREHENSIVE AND INTEGRATED TRANSITION UP PROGRAM

Program Evaluators purposefully, systematically, and carefully collect and analyze real-time information to document the effectiveness and impact of the Transition UP Program. This provides an objective means by which students and parents can assess progress. Through Program Evaluation students learn to engage in self-advocacy based on meaningful information as they gain greater autonomy and self-reliance.

For those who demonstrate no academic challenges, but struggle with social issues the Transition UP Personal Coaching Program can make a significant impact. This Program helps students focus upon social, attitudinal and communication objectives as part of the student’s Individualized Person-Centered Plan.
Each student is assigned a Personal Coach. Coaches are learning and behavior specialists who enable students to better set and achieve their goals. Coaches provide new ways to learn and practice vitally important skills that lead to success. For example, students learn to advocate for themselves so that they gain a sense of empowerment over their educational experience. In so doing students learn to seek out their own learning accommodations and address their individual learning differences. While self-advocacy is critical, sometimes coaches step in to fill the gaps that exist between the academic institution and the attainment of academic goals.

Coaching supports students to identify study strengths and needs, and to build an efficient and flexible set of study skills. More specifically coaching supports skills such as time management, remedies for procrastination, test preparation, note taking and reading strategies.
Coaches help to nurture a healthy educational experience. Coaches coordinate social support services; work directly with parents, schedule and facilitate both school-based on community-based social learning experiences. Finally, coaches serve as the liaison between the students and their families to facilitate and understand changes in the student’s personal development.

Behavior Consultants individually and in small groups support students to adjust their behavior to transition from one educational experience to the next and ultimately onto a career culture.
Learning Specialists and Tutors provide help to strengthen the student's ability to learn. The student engages in activities that teach processing, focusing, and memory skills. Assistance is offered to help students attain proficiency in prerequisite foundational skills such as reading, writing, mathematics, and oral expression, as well as support for specific course content throughout a student’s academic career. Scheduled weekly, study halls led by educational specialists provide the ideal setting for individual and small group tutoring.
Specialist in a variety of subjects assist with homework assignments, provide supplemental instruction and foster good study and executive habits. Depending upon the student's needs, tutoring is prescriptively provided by Program staff.



Completion of each rung on the school ladder, for many students, does not translate into true readiness to move on to the next stage of learning. For such students the Transition UP Year to Prepare Program is perfect. The Year-to Prepare Program provides a one-year program of specialized course work in academic planning, study skills development, remedial language, arithmetic basics, daily organizing, and anxiety management. Students remain enrolled in their school while concurrently attending the Year-to-Prepare Program.
Transition UP is a comprehensive educational/support program for primary, secondary and college students who are struggling to fit into their educational setting. It is completely integrated into the learning experience. Transition UP seamlessly supports students, while addressing their social skills, executive functioning, and personal development. The comprehensive nature of the Program, including individual instruction, coaching, and group practice provides for a true ownership of behavioral, attitudinal, and cognitive change.
